2002 OLP MITE A Hockey Team

OLPHC MINI-MITES FALL 2009 REGISTRATION:

To officially launch our teams this season there are a few items that parents / guardians must complete for each skater. We will need you to complete a skater registration package, pay the OLPHC 2009 registration fee and first month's ice fee.

SKATER REGISTRATION PACKAGE:

Download the OLPHC Registration Forms (under the links tab) from this website, print out, complete and submit a set for each skater (Please see the downloads section for paperwork)
Go to www.usahockeyregistration.com and register your skater(s) (this will take 1 minute)
Print-out a copy of the above registration form when completed and submit
Provide a copy of the skater's birth certificate
Provide a check for OLPHC Fall 2009 Registration (TBD)
Provide a check for August Ice Fees (see below)

MONTHLY ICE FEES:

All checks should be made payable to OLP2002.

Registration packages and payments should be received no later than August 15, 2009.

All documentation and payments should be given to the 2002 Mite A Team Manager, Eric Yeloushan.
